Jackson-Feild Homes is committed to respecting the privacy of our donors. We created this Donor Privacy Policy (“Policy”) to maintain the trust and confidence of donors and prospective donors. Jackson-Feild does not use or disclose sensitive personal information, such as race, religion, or political affiliations, etc.

By submitting your information to us or any of our staff, you consent to the terms and conditions of the Policy and to our processing personal information for the purposes stated below.
How your Information is Used. Jackson-Feild uses your data to complete and acknowledge donations and to communicate and update you on happenings on our campus. We maintain records of donations, gift acknowledgements, send newsletters, notices and solicitations and for internal fund raising purposes including responding to inquires.
Use and Disclosure of Personal Information
Donor Recognition. We do not list individual names in any of our publications. Corporate and foundation donors may be listed in our newsletter or annual report with their permission.
No Sharing of Personal Information. Jackson-Feild does not sell, rent, trade or share your personal information to any outside organization. We require third-party mailing services to treat our list as confidential information and are prohibited from transferring or utilizing this information for their own benefit. Donor information is used to further the mission and help the children served by Jackson-Feild. Best effort security measures may not prevent all intrusions and JFH is not responsible for any damages or liabilities relating to security failures.

Personnel and student files contain protected health information (PHI) and sensitive material which may cause detriment to the organization and to the person(s) to whom the information relates if this information is released to persons who are not legally authorized to obtain this information. The records covered by this policy include, but are not limited to:
Master Resident Files Departmental Resident Files Discharged Resident Files Any material pertaining directly to a resident or her/his family which has not been placed in the appropriate file to date, or which has been temporarily removed from the appropriate file.
Employee Personnel Files
These materials are protected by the Privacy Act of 1976 of the Commonwealth of Virginia and Title II of the Health Insurance Portability and Accountability Act of 1996.
Both legal and professional standards mandate that information be used for the delivery of services for children and their families. The proper maintenance of confidential information is crucial to the establishment of a trusting relationship between employees, residents, families, and the agencies Jackson-Feild Behavioral Health Services (JFBHS) serves.
Therefore, information covered by this policy must be kept in the strictest confidence and must never be discussed outside the work setting, even allegorically.
The employee understands that confidentiality is a condition of employment with JFBHS Health and that all confidential information will remain confidential in compliance with the organization’s policies and procedures and all applicable state and federal laws and regulations.
The only exception would be if failing to release the information would endanger the life, health, and/or safety of any human being, or would seriously jeopardize an on-going treatment plan.
Information contained in resident files and personnel files will not be reproduced unless the appropriate releases have been obtained. Copies will be marked “confidential” on all pages.
WHEN IN DOUBT, DO NOT RELEASE INFORMATION. CONTACT THE PRIVACY OFFICER FOR A SECOND OPINION.
The employee shall not, at any time during or following employment with JFBHS, disclose or use, except as required in the course of employment, any confidential information of JFBHS whether such information is in memory or embodied in writing or other physical form. Upon termination of employment, the employee shall return all such property received in connection with their employment and further agrees not to disclose any confidential information after they are no longer employed with JFBHS.

The residents’/clients’ clinical/medical record is the property of Jackson-Feild Behavioral Health Services and it is strictly confidential. Each resident/client will be afforded protection against the unnecessary disclosure of her/his identity and of personal information contained in her/his records. An authorization from the resident/client and/or legal guardian must be obtained in order to release resident/client information.
Resident/client information is defined as any written, computerized, or verbal information of a clinical, medical, or social nature regarding any resident/client, current or discharged, of this organization.
The clinical/medical record will not be taken outside of the facility.
In no instance are any copies of clinical/medical records to be made without the permission and specific approval of the Chief Operating Officer in consult with the Chief Executive Officer.
The record is not to be used in any manner which may jeopardize the resident’s interest.
Residents’/clients’ records are kept in the Medical Records Department.
Intentional or unintentional release of confidential information, verbal, computerized, or written, to an unauthorized person or organization shall result in disciplinary action up to and including discharge.
